2. Notarized Personal Data Sheet (CSC Form No. 212, Revised 2017) with recent passport-sized picture;
3. Latest Individual Performance Commitment and Review Rating or Performance Rating/Appraisal/Certification (if applicable);
4. Photocopy of training and seminar certificate;
5. Photocopy of certificate of eligibility/rating/license; and
6. Photocopy of Transcript of Records.
